We seek to help our clients throughout the Atlanta area enjoy their life at home, help them remain independent, and improve their overall quality of life through focused and attentive care.
Our team is composed of compassionate and highly trained care providers, including Nurses, Certified Nursing Assistants, Home Health Aides, and other certified professionals who are passionate and dedicated to helping our clients achieve their full potential in health and life.

Citadel Community Health Services (CCHS) is a Georgia-licensed home care provider serving clients in Atlanta and surrounding counties. CCHS is a Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD) provider. We are also a licensed Elderly and Disabled Waiver Program (EDWP) provider administered by the Department of Community Health (DCH).
