Citadel Community Health Services offers services for individuals with developmental disabilities and their families within the metro Atlanta area and surrounding counties. Services are authorized through the Division of Behavioral Health and Developmental Disabilities (DBHDD). The individual’s services are based on the service descriptions identified through the NOW & COMP waiver which is a Medicaid program that provides individuals with developmental disabilities the opportunity to receive services within their community.

Citadel Community Health Services is currently offering the following services under the NOW & COMP Waiver:

  • Community Living Support (CLS)- The Community Living Supports (CLS) program is designed to enable individuals with disabilities to lead an independent life free from the institutional atmosphere.
  • Individualized Community Access (CAI)- Community Access Individual (CAI) services are provided to an individual participant, with a one-to-one staff-to-participant ratio. CAI services are directly linked to goals and expectations of improvement in skills.
  • Group Community Access (CAG)- Community Access Group (CAG) services are provided to groups of individuals, with a staff-to-individual ratio of one to two or more. CAG services are designed to provide oversight and assist with daily living, socialization, communication, and mobility skills building and support in a group.
  • Nursing – These skilled nursing services promote health and prevent further deterioration of the health of an individual.
  • Specialized Medical Equipments & Supplies (SME & SMS)- Consists of food supplements, specialized clothing, incontinent supplies, and other equipment related to the individual’s disability.
