Items Needed When Submitting An Application

To process your application, you will need to provide us with proof of the following items.

  1. Reliable transportation
  2. Photo ID (preferably your driver’s license)
  3. Valid Car Insurance (Personal Support Aides)
  4. Valid GA Driver’s License (Personal Support Aides)
  5. Current RN, LPN, CNA Certification or DHR License
  6. Current CPR and First Aid card
  7. Social Security card or proof of number
  8. High School Diploma or GED
  9. Current physical (taken within the past year)
  10. Copy of PPD test or chest X-ray
  11. Six personal or professional references*

Before you can begin working for CCHS, you will also have to pass a criminal background check and drug screen.

***The references must include full names, addresses, phone numbers, and fax numbers (if available).
