    1. Confirm VA Health Care Enrollment & Eligibility

      Veterans must be enrolled in VA health care and meet Community Care eligibility criteria—such as lack of needed services nearby, long drive times, or clinical necessity. A VA staffer or primary care provider will identify needs and confirm eligibility.

    2. Clinical Assessment & Referral

      Based on a geriatric or clinical evaluation (including ADLs and IADLs), VA care teams determine which program fits (e.g., Homemaker/Home Health Aide, Skilled Home Health, Home-Based Primary Care, or Veteran-Directed Care) va.gov

    3. VA Refers to a VA-Approved Agency

      Once eligible, veterans are referred to a community care agency in the VA’s CCN—managed regionally by Optum or TriWest—or via a Veterans Care Agreement with aspecific VA facility. This is where we come in. You may indicate CitadelCommunity Health Services as your provider of choice. Speak with your VA casemanager to request authorization

    4. CCHS Provides In-Home Care

      Depending on the program, services may include aide visits, skilled nursing, therapy, or primary care teams delivered in the veteran’s home
