Our Story

Citadel Community Health Services (CCHS) was born in 2019 out of a deeply personal journey. Our founder witnessed firsthand the emotional and physical toll caregiving took on her grandmother, who cared for her terminally ill husband. That experience opened her eyes to the overwhelming challenges many families face when caring for loved ones, often without adequate support.

Determined to make a difference, she launched CCHS to provide compassionate, dignified, and dependable care. Today, Citadel Community Health Services provides in-home personal support care and nursing services to individuals across Metro Atlanta, serving the community with a deep commitment to person-centered care, family collaboration, and clinical excellence.

Our Primary Mission

Our home care agency's primary objective is to enhance the lives of seniors, veterans, and individuals with intellectual and developmental disabilities. To achieve this goal, we provide a comprehensive range of services—from friendly companionship and homemaking to meal preparation and skilled nursing care.

We carefully select each of our caregivers to ensure the safety, comfort, and peace of mind of our clients and their families. Every caregiver undergoes a thorough interview process that includes background checks, drug and alcohol screening, and reference verification.

In addition, we thoughtfully match seniors with compassionate caregivers who have the appropriate experience, qualifications, and personal characteristics to provide exceptional companionship and care. Our goal is to foster meaningful, long-lasting relationships between caregivers and the individuals they serve.
